After you read the forms, please print necessary pages, fill out and sign them. Retain copies for your records. Bring the completed and signed pages with you to your first visit.  Thank you 🙂

Rate:  $135 for 50 minute session

Insurance:   You may have mental health coverage for counseling services, so it’s important to check with your provider and ask them the questions listed below.  I am not currently on any insurance panels, however you could still be reimbursed some amount through “out of network” benefits.  This is done by taking the requested receipt from your session and filing with your insurance provider.
I do accept some  “flexible benefit” cards.  If you have an insurance savings account card, check with your company to determine if my services are covered.

  • what are my “out of network” mental health benefits?
  • Do I have a deductible and/or copay for my out of network provider?
  • Is there a certain number of sessions allowed and is precertification necessary?
  • How do I submit my claim form?  By mail, fax, email?

Forms of payment accepted:  Major credit cards, check, and cash.
